To ensure we have all your information and an accurate medical history, we encourage our patients to pre-fill their new patient registration form. Click to download a copy of the form. Payment due on the day

Payment of your account is due on the day of your treatment. In certain cases a deposit may be required before your treatment has commenced.

Special payment arrangements

To help you receive the best treatment options for your needs, we're happy to discuss other payment arrangements with you as necessary for larger treatment plans. Please talk to us about this before your treatment has commenced.


APPOINTMENT TIMES

Courtesy reminders

We will provide a courtesy reminder 48 hours before your appointment time via SMS, email or phone call.

Changing your appointment

If you need to change your appointment time, we kindly ask you to give us at least 48 hours notice so that we can use that time for another patient who needs it.

Missing an appointment

If you fail to attend your scheduled appointment without notifying us, it may result in an invoice reflecting the amount of time that has been set aside for you.
